Cerebellar nystagmus is the tremor of the eyeballs that occurs usually when one attempts to fixate the eye on a scence to one side of head. This is off center type of fixation result in rapid tremerous movement of the eyes. Nystagmus is an involuntary “to and fro” movement of the eyes in a horizontal, vertical, rotatory or mixed direction. Nystagmus may be pendular (equal velocity and amplitude in all directions) or jerk (with a fast phase and a slow phase).
Nystagmus may result from :- 1) Retinal disease 2) Labyrinthine disease 3) Disorders affecting the cerebellum or substantial portion of the brain stem. Nystagmoid movements of the eyes are present in many people at extreme gaze. It occurs when the flocculonodular lobes are damaged, in this instance it is associated with the loss of equilibrium because of dysfunction of the pathways through the flocculonodular cerebellum from the semicircular ducts.
Spontaneous horizontal nystagmus with the quick component towards the side of lesion occurs with asymmetric lesions in the posterior vermis, flocculonodular lobe & nucleus fatigi. Retinal nystagmus occurs when vision is defective. Fixation is impaired and the eyes vainly search mostly occurs in congenital cataract, congenital macula defect, albinism. Vestibular nystagmus creates an imbalance between each side resulting in a slow drift of the eye towards the damaged side followed by a fast compensatory movement to the opposite side.
